A principios del siglo XI se decide realizar una arqueta que contenga las reliquias de San Millán. La primera, fechada en torno a1030, parece no reunir los requisitos para albergar tan insignes restos, de manera que rápidamente se decide confeccionar una segunda, la cual está concluida en 1067, cuando se procede a depositar los restos del santo. Para su ejecución se utilizó madera, oro, piedras preciosas y placas de marfil, en las que se contaban historias relativas a la vida de San Millán. La historia de la arqueta continúa, pues en el siglo XV fue reformada, aunque más grave fue el expolio a la que fue sometida en el año 1809 por los soldados fraceses, que la despojaron de los marfiles, piedras preciosas y metales. Recientemente se ha llevado a cabo una minuciosa reconstrucción de lo que pudo ser la pieza.
La arqueta presenta una estructura rectangular cubierta por una tapa a doble vertiente. Uno de los frontales está presidido por el Pantocrator rodeado por el Tetramorfos, el original conservado en Nueva York. El segundo frontal presenta la muerte del santo y culmina todo un ciclo de la vida de San Millán que se dispone en la diferentes tablas de marfil que rodean la arqueta. Entre las escenas podemos ver a el santo con su discípulos, la predicación de San Millán de Cantabria, donde se representa la famosa escena de la caida de este territorio a manos de Leovigildo.
